We visit you at your home for an initial consultation at a time and date that suits you to complete a comprehensive assessment in your own home or a mutual place that suits all parties concerned. We would then discuss a bespoke care package and agree outcomes in a way that makes sense to the individual. We co-create person-centred support plans with each individual, their families and the wider support network.
The relationships that we build with our clients and their families, carers are central to ensuring a seamless transition and the application of a personalised care package. We will take into consideration your background, health conditions, daily routine, lifestyle, preference, wishes and needs.
